A C-peptide test is one of the most reliable tools endocrinologists use to figure out exactly what is happening inside your pancreas, helping to firmly distinguish between Type 1 and Type 2 diabetes.
When your pancreas produces insulin, it initially creates a larger molecule called proinsulin. Before this molecule is released into your bloodstream, it splits into two equal pieces: one piece is insulin, and the other piece is C-peptide. Because they are released in perfectly equal amounts, measuring your C-peptide tells your doctor exactly how much of your own insulin your body is naturally producing.
You might wonder why they don't just measure insulin directly. First, if you are already injecting synthetic insulin, a standard blood test cannot easily tell the difference between the insulin you injected and the insulin you made. However, injected insulin does not contain C-peptide! Second, natural insulin clears out of the bloodstream very quickly, whereas C-peptide sticks around longer, making it much easier to measure accurately.
If your C-peptide levels are extremely low or practically zero, it confirms that your pancreas is no longer making insulin, pointing toward Type 1 diabetes (or LADA). If your C-peptide levels are normal or unusually high, it means your pancreas is pumping out plenty of insulin, but your body is resistant to it, confirming Type 2 diabetes.
